Job summary

Mount Evans Hospice is seeking a Registered Nurse - PRN to provide hospice care in patients' homes around Evergreen, CO. The role offers flexible, as-needed hours to support regular staff during time off or increased demand.

The RN will assess patients, plan and implement nursing care, and collaborate with families and the care team. Current state nursing licensure and CPR are required, with home health experience preferred.

Qualifications

• Current licensure in the state and CPR certification upon hire.
• Ability to perform comprehensive nursing assessments and develop care plans.
• Experience in home health or hospice nursing preferred.

Responsibilities

• Performs initial and ongoing patient assessments to determine hospice needs.
• Delivers professional nursing care per the nursing process and state act.
• Develops and initiates care plans with patient and family involvement.
• Documents clinical notes within 48 hours and communicates with the physician.
• Participates in on-call duties and coordinates with the care team.
