Job summary

CarePartners Health Services in Asheville, NC seeks a dedicated Primary Case Manager RN in Hospice to lead nursing care and clinical case management, coordinating with the interdisciplinary team to align care with patient goals.

You will assess, plan, implement, and adjust care plans, support families through end-of-life transitions, and promote quality of life within culturally sensitive palliative care.

Qualifications

• Required Education: Diploma in Nursing, Associate’s Degree in Nursing, or Bachelor’s Degree in Nursing.
• Required License: Must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing; compact license may apply; BCLS; valid Driver’s License.
• Required Experience: One year acute care experience.

Responsibilities

• Provide nursing care and clinical case management to patients and families.
• Coordinate services with hospice interdisciplinary team.
• Perform comprehensive assessments and update plans of care with patient/caregiver.
• Support transitions through end-of-life care with culturally sensitive communication.
