Responsibilities

• Provides medical services during patient home visits (assessment, treatments, recertifications, health teaching).
• Manages crisis situations and follows through.
• Ensures treatments are medically appropriate and cost‑effective.
• Collaborates with Hospice Medical Director and team on symptom management.
• Confirms patient eligibility for Hospice benefits per regulations; certifies/re-certifies eligibility.
