Horizon Home Care & Hospice is seeking a dedicated Nurse Practitioner to serve as a Hospice Case Manager (Patient Care Coordinator) in the Milwaukee area. You will conduct home visits, coordinate care with an interdisciplinary team, and ensure comfort for patients at home.

Qualifications

• Current, valid Nurse Practitioner license in Wisconsin
• Valid driver’s license and reliable transportation
• Strong documentation skills with EMR systems
• Commitment to hospice philosophy and patient-/family-centered end-of-life care

Responsabilités

• Perform face-to-face patient visits and document findings for physician review
• Educate clinical staff and physicians on identified topics and pain management
• Collaborate with interdisciplinary team to plan and adjust care
• Provide nursing visits as required, including end-of-life comfort measures
• Prescribe and review medications under Medical Director guidance
• Support on-call coverage to meet patient needs 24/7 as scheduled
