Qualifications

• Current, valid Nurse Practitioner (NP) license in good standing in Wisconsin.
• Strong documentation skills with ability to learn electronic medical records systems.
• Commitment to hospice philosophy and patient- and family-centered end-of-life care.

Responsibilities

• Conduct face-to-face encounters and document findings for certifying physician.
• Provide medical services including assessment, treatments, and health teaching during home visits.
• Consult with attending physicians regarding palliative and terminal illness management.
• Document and communicate encounter findings to supervising physicians in a timely manner.
• Support staff by identifying patient issues and coordinating care with the interdisciplinary team.
• Prescribe and review appropriate medications under Medical Director supervision.
• Collect and analyze patient data to inform care plans.
