Job summary

VITAS Healthcare in Chicago, IL seeks a compassionate Home Care Hospice Physician to join our nationwide leader in end-of-life care. You will build patient-centered care plans, work with an interdisciplinary team, and deliver the highest quality comfort and dignity in patients' homes, with visits in person or via telehealth.

You must be licensed to practice medicine in the state, board-certified or eligible in hospice and palliative medicine or related specialties, and familiar with hospice

Qualifications

• Licensed to practice medicine in Illinois.
• Board-certified or board-eligible in hospice and palliative medicine, internal medicine, family practice, or related specialty.
• Familiar with hospice principles; DEA registration.

Responsibilities

• Provide high-quality end-of-life care in patients' homes.
• Manage care through an interdisciplinary team approach.
• Conduct visits in person and via telehealth.
• Participate in weekly IDG meetings.
• Educate patients and families about goals of care.
