Job summary

Qualifications

• Must be licensed to practice medicine or osteopathy in California.
• Board-certified or board-eligible in hospice/palliative medicine, internal medicine, family practice, or related specialty.
• DEA registration and familiarity with hospice principles.

Responsibilities

• Provide in-home visits and telehealth to patients.
• Lead an interdisciplinary team to develop and implement care plans.
• Participate in weekly IDG meetings and advocate for patient goals.
