Job summary

VITAS Healthcare is seeking a home care hospice physician to provide compassionate, patient-centered end-of-life care. You will create care plans tailored to each patient’s wishes and collaborate with a dedicated interdisciplinary team to support families.

You will see 45–60 patients in their homes, with visits conducted in person or via telehealth. Weekly IDG meetings are part of the routine to ensure coordinated care.

Qualifications

• Licensed physician in Missouri with board certification or eligibility in hospice/palliative medicine, internal medicine, family practice, or related specialty.
• Familiarity with hospice principles and ability to coordinate with patient’s primary physician.
• Experience in managing care plans centered on patient wishes and goals.
• Knowledge of symptom control in terminal illness.
• Registration with DEA to write scheduled prescriptions.

Responsibilities

• Build meaningful relationships with patients and families, delivering high-quality care in home settings.
• Manage 45–60 patients via interdisciplinary team, with in-person and telehealth visits.
• Participate in weekly IDG meetings to coordinate care.
• Advocate for patients’ goals and educate families on end-of-life care benefits.
