Job summary

VITAS Healthcare is seeking a hospice physician to provide compassionate, patient-centered end-of-life care in patients' homes. You will see the person behind each prognosis and collaborate with an interdisciplinary team to create patient-centered care plans.

You will manage 45-60 patients, perform both in-person and telehealth visits, and participate in weekly IDG meetings. Must be licensed, with board certification/eligibility in hospice, internal medicine, family practice, or related

Qualifications

Licensed to practice medicine or osteopathy in the state of operation.
Board-certified or board-eligible in hospice and palliative medicine, internal medicine, family practice, or related specialty.
Registered with the DEA.

Responsibilities

Provide high-quality end-of-life care to patients in home settings as part of an interdisciplinary team.
Manage approximately 45-60 patients and coordinate care plans.
Conduct in-person and telehealth visits to patients and families.
Participate in weekly interdisciplinary team (IDG) meetings.
Advocate for patient goals, addressing hopes and fears with compassion.
Educate patients, caregivers, and communities about end-of-life care benefits.
