Job summary

Gentiva Hospice is seeking a Hospice Physician to ensure compassionate, goal-directed care for patients within our hospice program. You will collaborate with interdisciplinary teams, oversee regulatory compliance, and contribute to quality improvement initiatives that enhance patient outcomes.

This is a flexible/per diem role, not directly employed by Gentiva; you will invoice for services rendered and may travel as program needs dictate to support patient care and program operations.

Qualifications

• Doctorate of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited school.
• Board certification in ABMS or AOA-BOS preferred.
• Current unrestricted medical license in practicing state.
• Valid NPI and DEA registration.
• Hospice/Palliative Medicine board cert or Hospice Medical Director cert preferred.
• Current Medicare PECOS and Medicaid certification if required.
• Ability to travel 15-25% as needed.
• CPR and TB clearance required.

Responsibilities

• Certify and recertify hospice eligibility in compliance with Medicare and regulations.
• Conduct and supervise home visits and complex case consultations.
• Provide 24/7/365 on-call coverage coordination.
• Maintain timely medical documentation for regulatory reviews.
• Participate in IDG meetings to coordinate care and meds.
• Monitor pharmacy utilization and appropriate prescribing.
• Collaborate with leadership on policy, QAPI, and quality improvement.
• Serve as community liaison to enhance hospice partnerships.
• Support staff education for clinicians and partners.
