Job summary

Gentiva Hospice is seeking an experienced Hospice Physician to join our leadership team. This role focuses on certifying eligibility, coordinating complex patient visits, and guiding interdisciplinary care to ensure compassionate, compliant end‑of‑life services.

You will provide 24/7 on‑call coverage, mentor staff, collaborate on policy and QAPI initiatives, and build strong partnerships with community providers. Travel of 15–25% is expected as part of program needs.

Qualifications

• MD/DO with board certification in hospice or primary care.
• Current unrestricted license in practicing state.
• NPI and DEA registration for controlled substances.
• Willingness to travel 15–25%.
• CPR and TB clearance required.

Responsibilities

• Certify and recertify hospice eligibility under Medicare rules.
• Conduct and supervise face-to-face patient visits, incl. home visits.
• Provide 24/7 on-call physician coverage coordination.
• Maintain timely medical documentation and support reviews.
• Participate in IDG meetings to coordinate care.
• Monitor pharmacy utilization and appropriate prescribing.
• Collaborate on policy development and QAPI initiatives.
• Serve as a community liaison to boost partnerships.
• Support educational programs for staff and partners.
