Job summary

OnePoint Patient Care in Las Vegas, NV is seeking a Pharmacist for a full-time, closed-door hospice pharmacy role on a 7:00pm–7:00am schedule (7 days on, 7 days off). You will review prescriptions, ensure safety, and collaborate with clinicians to optimize patient therapy.

You’ll maintain patient profiles, provide drug-therapy recommendations, and supervise IV preparation and automated dispensing systems. Active license in NV required; PharmD preferred.

Qualifications

• Bachelor's Degree in Pharmacy; PharmD preferred.
• Current and valid pharmacist license in state of practice.
• Knowledge of drug therapy guidelines and regulatory standards (CMS, DEA, FDA).
• Proficiency with electronic medical records and pharmacy information systems.
• Strong clinical judgement and clear written and verbal communication.

Responsibilities

• Receive and evaluate prescriptions for drug appropriateness, correct dosage, interactions, and route.
• Collaborate with physicians, nurses, and other healthcare professionals for clinical support.
• Maintain up-to-date patient profiles including medical history and medications.
• Provide clinical recommendations to physicians and hospice nurses regarding therapy changes.
• Implement and enforce policies for safe procurement, storage, administration and disposal of meds.
• Ensure compliance with state and federal regulatory requirements for pharmacy practice.
• Oversee tasks such as narcotics room duties, supervising automated prepackaging, IV prep, and Omnicell systems.
• Perform other pharmacy-related tasks as assigned.
