Job summary

OnePoint Patient Care in Texas is seeking a Pharmacist to join our hospice-focused team. You will dispense medications, provide clinical advice, and support delivery services.

We emphasize high-quality patient care and collaboration with nurses and physicians; schedule is Monday–Friday, 10:00am–6:00pm with possible weekend rotation and on-call responsibilities.

Qualifications

• Strong communication skills, customer service, and problem-solving abilities.
• Pharmacist licensure in good standing is required.
• Pharmacist degree or equivalent is needed.

Responsibilities

• Provides clinical consultations to patients and advises nurses, physicians, and hospice staff.
• Evaluates medication profiles, compliance, and treatment adjustments.
• Answers customer questions and resolves issues promptly.
• Retrieves data in the pharmacy system.
• Manages medication orders.
• Ensures proper storage and security of medications.
• Maintains an orderly, productive, and accurate work environment.

• Answers customer questions and resolves customer issues in a timely manner
• Retrieves data in the pharmacy system
• Manages medication orders
• Ensures proper storage and security conditions for medications
• Maintains a productive, orderly, and error-free work environment

Qualifications

• Doctor of Pharmacy Degree or Bachelor of Science Pharmacy Degree
• Current and good standing Pharmacist License
• Strong communication skills, customer service, and problem-solving skills
