OnePoint Patient Care in Columbia, MD, is hiring a full-time Pharmacist to join a non-retail hospice pharmacy team. You will review prescriptions, collaborate with physicians and nurses, and maintain up-to-date patient profiles in a supportive environment.

The role offers a Monday–Friday schedule (10:00am–6:00pm) with on-call rotation for holidays, comprehensive benefits, and opportunities for career growth and advancement within the company.

Qualifications

• Pharmacist degree and state license required.
• Pharmacist license must be current and valid in-state.
• Pharmacists should be proficient with EMR and pharmacy systems.
• Clinical judgement and clear communication are essential.

Responsibilities

• Review prescriptions for drug appropriateness and dosage.
• Collaborate with clinicians to optimize therapy for hospice patients.
• Maintain patient profiles with current medications and history.
• Ensure compliance with state and federal pharmacy regulations.
• Support IV prep, narcotics room, and automated dispensing technologies.
