OnePoint Patient Care is seeking a full-time Pharmacist to join our closed-door hospice pharmacy team in Tulsa, OK. You will evaluate prescriptions, collaborate with healthcare professionals, and manage patient profiles to ensure safe, effective medication therapy for hospice patients.

The role offers ongoing training, opportunities for advancement, and a supportive team with the potential for remote work flexibility as appropriate.

Qualifications

• Bachelor’s Degree in Pharmacy; PharmD preferred
• License/Certification: current and valid pharmacist license in the state of practice
• Knowledge of drug therapy guidelines and regulatory standards (CMS, DEA, FDA)
• Proficiency in EMR and pharmacy information systems
• Strong clinical judgement and communication skills
• Position posted for a minimum of 7 business days

Responsibilities

• Receive and evaluate prescriptions for drug appropriateness, dosage, interactions, and administration route.
• Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
• Maintain up-to-date patient profiles, including medical history and current medications.
• Provide clinical recommendations to physicians and hospice nurses regarding medication therapy changes.
• Implement and enforce policies for safe procurement, storage, administration, and disposal of medications.
• Ensure compliance with state and federal regulatory requirements related to pharmacy practice.
• Perform specialized tasks such as supervising automated machines, IV medications, and facility-based dispensing systems (e.g., Omnicell).
• Perform other pharmacy-related tasks as assigned.
