Job summary

Hospice & Palliative Care Buffalo seeks a Patient Services Specialist (Intake) to coordinate intake, scheduling, and care delivery for patients in Buffalo, NY. You will interview patients, compile information, and make referrals as needed. Strong data entry into the EMR system and insurance verification are essential.

The role runs 8:30am-4:30pm, Monday-Friday with weekend rotation and holiday rotation. Salary ranges $21.75-$24.00 per hour for FT status.

Qualifications

• High School Diploma or GED required.
• Experience in healthcare environment preferred.
• Experience in Hospice & Palliative Care helpful.

Responsibilities

• Coordinate intake, scheduling, and care delivery for patients.
• Conduct interviews to collect intake information.
• Refer patients to service providers based on needs and assessments.
• Enter patient information and calls into EMR system.
• Assist in day-to-day coordination of team clinical functions and assessments.
• Verify insurance information and benefit eligibility.
