Job summary

Mass Digital Health in Quincy is seeking a part-time Licensed Practical Nurse to assist in the Admissions Department. This role includes coordinating patient care, scheduling visits, and collaborating with interdisciplinary teams.

The ideal candidate will have recent nursing experience, New York LPN licensure, and strong communication skills. The position requires availability every Saturday and Sunday from 8 a.m. to 4 p.m., with holiday rotation.

Qualifications

• Graduate of an accredited School of Practical Nursing with current LPN licensure in New York.
• 1 year of nursing practice as a Licensed Practical Nurse preferred.

Responsibilities

• Assists in coordination and implementation of the patient plan of care.
• Handles phone calls from patients or caregivers regarding questions or concerns.
• Collaborates with interdisciplinary teams, RNs, or Team Manager.
• Coordinates and schedules patient visits.
• Provides timely documentation/data entry.
• Communicates current health status and needs of patients.
• Assists the team and Team Manager with administrative functions.

Shift & Schedule

This is a part‑time position in the Admissions Department. 8 a.m.–4 p.m. every Saturday and Sunday, with holiday rotation.

Essential Duties & Responsibilities

• Assists in coordination and implementation of the patient plan of care.
• Handles phone calls from the patient, primary caregiver, or designee regarding any questions or concerns.
• Collaborates with the interdisciplinary teams, RNs, or Team Manager.
• Coordinates and schedules patient visits.
• Provides timely documentation/data entry per Hospice time frames and deadlines.
• Communicates current health status and needs of patients to appropriate team professionals.
• Assists the team and Team Manager with other administrative functions, as necessary.

Qualifications

• Graduate of an accredited School of Practical Nursing with current LPN licensure in the State of New York.
• Experience: 1 year of nursing practice as a Licensed Practical Nurse preferred.
