Job summary

Moments Hospice in Madison, WI is seeking a dedicated RN Case Manager to lead clinical care for hospice patients and families. You will assess needs, manage symptoms, and coordinate services to ensure comfort and dignity at the end of life.

The role is field-based with a company fleet vehicle program to support home visits. You will work closely with clinicians and leadership to balance caseloads and deliver compassionate care.

Qualifications

• Licensed RN with active Wisconsin license.
