Job summary

Bristol Hospice is seeking a Hospice Social Worker in Eugene/Springfield, OR. The role involves assessing psychosocial status, counseling patients and families, and coordinating with the interdisciplinary team to support end-of-life care.

Requirements include a Master’s degree in social work and at least two years of supervised health care experience; hospice experience is preferred. Some travel within the service area is expected.

Qualifications

• Must have a master’s degree in social work from an accredited university.
• Minimum of two years of documented supervised experience in health care, hospice experience preferred.
• Must understand hospice philosophy, and issues of death/dying.
• Certified Hospice and Palliative Social Worker is desirable.
• Must be flexible in work hours and able to travel within the Bristol Hospice service area.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers related to terminal illness and communicate findings to the care team.
• Provide psychosocial assessments at patient residences and plan interventions as needed.
• Carry out social evaluations including family dynamics, caregiver abilities, and risks, then plan interventions.
• Counsel patients and families to help with stress and coping, offering crisis intervention as necessary.
• Maintain clinical records for all patients referred to social work and educate on advance directives.
