Job summary

Pallatus Health is seeking a board-certified Hospice and Palliative Care Physician to lead our interdisciplinary care team in delivering compassionate end-of-life care in home and facility settings.

You will assess patients, develop individualized care plans, manage pain and symptoms, mentor staff, and participate in quality improvement initiatives, while ensuring regulatory compliance and patient-centered communication.

Qualifications

• MD or DO from an accredited school.
• Board certification in Internal Medicine, Family Medicine, or related specialty.
• Active state medical license and DEA registration.
• 2–3 years of clinical experience, preferably in hospice or palliative care.

Responsibilities

• Conduct comprehensive medical assessments and develop personalized hospice care plans.
• Manage pain and symptom control with expertise in palliative medicine.
• Lead interdisciplinary team meetings and coordinate care with nurses, social workers, chaplains, and other specialists.
• Provide on-call coverage and respond to urgent patient needs in home and facility settings.
• Communicate prognosis, care options, and end-of-life planning with patients and families.
• Document encounters thoroughly and ensure regulatory compliance.
• Mentor team members and educate patients and families on hospice and palliative care principles.
• Engage in quality improvement initiatives and community outreach.
