Job summary

Gentiva Hospice is seeking a PRN Nurse Practitioner to provide dual hospice and palliative care, delivering patient-centered management and Face-to-Face visits for eligibility. You will work with IDG/IDT, manage symptoms, and support families through end-of-life planning.

The role blends autonomy with team collaboration, requiring strong clinical assessment, documentation, and compliance with Medicare/Medicaid requirements. Flexibility with patient census is expected.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum two years NP experience preferred.
• Experience in hospice/palliative care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Perform Medicare-required hospice Face-to-Face visits within CMS timeframes.
• Collaborate with IDG/IDT to deliver comprehensive, patient-centered care.
• Conduct patient assessments across physical, emotional, and psychosocial domains.
• Develop and adjust plans of care with patients, families, and caregivers.
• Prescribe medications and manage treatments within scope of practice.
