Job summary

Ideal candidates hold an MSN or higher, have NP certification, and bring experience in hospice/palliative settings. This role emphasizes regulatory compliance and compassionate care.

Qualifications

• Master of Science in Nursing (MSN) or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years of Nurse Practitioner experience preferred; hospice/palliative care experience preferred.

Responsibilities

• Perform patient management visits and hospice F2F encounters for eligibility certification and recertification.
• Prescribe medications and treatments within state law and Collaborative Practice Agreement or Independent Practice.
• Coordinate care with IDG/IDT and collaborate with physicians and specialists.
• Document visits accurately and attest F2F encounters in the medical record per regulatory standards.
• Manage all aspects of patient care including symptom management and end-of-life care.
