Job summary

The Battle Creek VA Medical Center is seeking an Advanced Practice Nurse (Nurse Practitioner) in the Hospice/Palliative Care unit. The NP provides evidence-based care to patients referred to palliative care/hospice clinics and consultations, functioning as a licensed independent practitioner within a collaborative team.

Responsibilities include managing a panel of patients, developing individualized care plans, ordering and interpreting labs, and educating patients and families while upholding

Qualifications

• Independently manages a panel of patients with acute and chronic conditions in palliative care.
• Prescribes pharmacologic and non-pharmacologic interventions and initiates referrals.
• Demonstrates leadership and collaborates with the interdisciplinary team.

Responsibilities

• Orders and interprets labs, tests, and diagnostic studies.
• Educates patients and families on care plans.
