Qualifications

• Current, active, and unrestricted RN and NP licensure in state of practice.
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care.
• Commitment to caring for patients and partners.
• Completes all documentation and records in a timely and accurate manner; follows established policies and procedures related to the scope of practice.
• Maintains continuity of patient care in a hospice setting.
