Job summary

PruittHealth Hospice is seeking a Nurse Practitioner (NP) for hospice and palliative care services. Join a team dedicated to compassionate patient care in the home or facility setting with top-of-market pay and comprehensive benefits.

Responsibilities include comprehensive adult/geriatric assessments, developing and managing care plans, timely documentation, and adherence to hospice policies to ensure continuity of patient care.

Qualifications

• Current, active RN and NP licensure in state of practice.
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care
• Commitment to caring for patients and partners
• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting
