Responsibilities

• Share our mission and services with compassion and clarity to referral partners.
• Build and strengthen relationships with hospitals, home health agencies, SNFs, clinics and physician offices.
• Engage in meaningful conversations with healthcare professionals about patient needs and hospice care benefits.
• Plan and carry out outreach efforts to raise hospice awareness and timely referrals.
• Represent Good Shepherd Hospice at community events and professional associations.
