Responsibilities

• Build and strengthen relationships with hospitals, home health agencies, skilled nursing facilities, assisted living communities, clinics, and physician offices.
• Engage in meaningful conversations with healthcare professionals about patient needs and hospice care benefits.
• Plan and carry out outreach efforts to increase awareness of hospice services and timely referrals.
• Represent Good Shepherd Hospice at community events, professional associations, and educational in-services.
