Job summary

Interim HealthCare Inc. in Ohio is seeking a Hospice Weekday On-Call Nurse (RN) to provide compassionate, patient-centered care when needs arise.

This full-time role covers Weekday On-Call shifts, Monday-Thursday 5pm-8:30am, with one-on-one patient care and a focus on comfort and dignity. You will collaborate with the hospice team to assess plans of care, update care plans, and communicate patient conditions to ensure timely and appropriate treatment.

Qualifications

• ADN or AAS required; BSN preferred.
• RN licensure in the applicable state.
• Current CPR/FA certification.
• One year of RN hospice or similar setting experience preferred.

Responsibilities

• Oversee the implementation and ongoing assessment of the patient's plan of care with the care team.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and coordinate with providers to adjust care as needed.
