Job summary

Three Oaks Hospice is seeking a PRN On-Call Registered Nurse (RN) to join our team serving Gwinnett, Forsyth, and Dekalb Counties. This position offers weekend coverage (Sat/Sun 9AM - 5PM) with a compassionate focus on hospice and palliative care.

Primary responsibilities include administering skilled nursing care in patients' homes, coordinating care with the interdisciplinary team, and supporting patients and families through transitions.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ RN experience; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support patients and families.

Responsibilities

• Administer skilled nursing care for clients in their residence.
• Coordinate care with interdisciplinary team, patient/family, and referrals.
• Ensure smooth coordination of care and documentation.
