Job summary

Three Oaks Hospice is seeking a compassionate On-Call Registered Nurse (RN) to join our hospice care team serving Forsyth, Gwinnett, and Dekalb counties. The RN will provide skilled nursing care in patients' homes, coordinate with the interdisciplinary team, families, and referring agencies, and participate in on-call rotations.

This is a weekend-centric full-time role with weekend coverage from Friday 5pm through Monday 9am.

Qualifications

Graduate of an accredited Associate or bachelor’s degree in Nursing.
Current state RN license and valid driver’s license.
Two years’ RN experience; hospice/home health preferred.
Current CPR certification.
Strong communication and interpersonal skills.
Proficient computer skills; able to coordinate care.

Responsibilities

Administer skilled nursing care in clients' homes.
Coordinate care with interdisciplinary team, patient/family and referring agency.
Provide support to patients and families; coordinate care across settings.
On-call responsibilities as part of rotation.
