Job summary

Three Oaks Hospice, Inc. is seeking a PRN On-Call Registered Nurse (RN) to join our hospice care team serving Clarke County and surrounding areas.

The role focuses on delivering skilled nursing care in patients’ homes, coordinating with the interdisciplinary team, families, and referring agencies. The successful candidate will have an RN license, CPR certification, and at least two years of clinical RN experience; hospice or home health experience is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current RN license and valid driver’s license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• CPR certification.
• Computer proficiency; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support patients and families.
• Demonstrate skilled nursing practice and coping with family emotional stress.

Responsibilities

• Administer skilled nursing care for clients in their residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
• Assist with care planning and documentation as part of the care team.

We are looking for a passionate PRN On-Call Registered Nurse RN to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY:

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.

QUALIFICATIONS:

• Graduate of an accredited Associate or bachelor’s degree in Nursing
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
