Job summary

Agape Hospice Care in Georgia is seeking a passionate PRN On-Call Registered Nurse to join our hospice team and deliver compassionate in-home care.

The RN will coordinate with the interdisciplinary team, manage skilled nursing tasks, and support patients and families in Clarke County and surrounding areas on Saturdays, Sundays 8a-5p and occasional weekdays. A Nurse with strong communication and clinical skills is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support the patient and family.
• Ability to coordinate nursing care with the interdisciplinary team.

Responsibilities

• Administer skilled nursing care in clients’ homes.
• Coordinate care with the interdisciplinary team, patient, family and referring agency.
• Assist with care planning and documentation.

We are looking for a passionate PRN On-Call Registered Nurse RN to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY: Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.

QUALIFICATIONS:

• Graduate of an accredited Associate or bachelor’s degree in Nursing
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
