Job summary

Agape Hospice Care is seeking a passionate PRN On-Call Registered Nurse (RN) to join our hospice team serving Clarke County and surrounding areas. The role requires weekend on-call shifts and occasional weekday PRN coverage, with duties focused on delivering compassionate, skilled nursing care in patients’ homes.

The incumbent will coordinate care with the interdisciplinary team, support families, and ensure adherence to care plans, with a strong emphasis on patient comfort and clinical

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient use of computers; MS Office experience preferred.
• Strong interpersonal skills and ability to support patients and families.
• Demonstrate skill in nursing practice and coping with family emotional stress.

Responsibilities

• Administer skilled nursing care for clients in their homes or residence.
• Coordinate care with the Interdisciplinary Team, patient/family, and referring agency.
• Assist with care coordination and communication as part of the patient’s plan of care.

We are looking for a passionate PRN On-Call Registered Nurse RN to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY: Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.

QUALIFICATIONS:
