Qualifications

• Active Nursing License in MO/KS.

Responsibilities

• Provide on-call nursing care to hospice patients in home or facility settings.
• Assess patient and caregiver needs via telephone, home visits, or facility visits and coordinate with the care team.
• Plan and direct services to meet patient and family needs after hours.
• Document care and communicate with interdisciplinary team to ensure quality of life and symptom management.
