CareerNerds, LLC is seeking an on-call Hospice Nurse (LPN or RN) for a 7on/7off schedule in the East Kansas City Metro area. Responsibilities include assessing needs, coordinating care, and providing patient-centered symptom management in homes and facilities after hours.

The role requires two to five years of hospice experience, active MO/KS nursing license, and CPR certification. Excellent communication and organizational skills are essential for success.

Competenze

• Active MO/KS nursing license required.
• CPR certification required.
• 2–5 years hospice/palliative care experience preferred.
• Knowledge of hospice services and interdisciplinary care coordination.
• Strong organizational, verbal and written communication skills.
• Analytical, detail-oriented with problem-solving abilities.

Mansioni

• Provide evening and weekend on-call nursing support via telephone, home visits, and facility visits.
• Assess patient and caregiver needs; coordinate and direct care within home/community settings.
• Educate patients and families on symptom management and care planning.
