Job summary

VitalCaring Group in Florida is seeking a Hospice Occupational Therapist to plan, organize, and deliver specialized therapy services that support comfort, function, and dignity at end of life. You will work within a collaborative care team to implement individualized plans of care guided by our core values.

You will evaluate functional status, adapt the home environment, educate families, supervise COTAs, and contribute to discharge planning.

Qualifications

• Graduate of an accredited occupational therapy program meeting professional standards.
• Registered through the National Registration Examination of the American Occupational Therapy Association.
• Minimum of two years of clinical experience as an Occupational Therapist.
• Strong verbal, written, and organizational skills.

Responsibilities

• Evaluate the patient’s functional status and activity tolerance.
• Assess home environment to identify barriers and necessary adaptive equipment.
• Develop individualized plan of care and treatment goals.
• Teach skills or retrain patients in activities of daily living.
• Fabricate splints and instruct on adaptive equipment.
• Provide training for prosthetic or orthotic devices as indicated.
• Educate family, caregivers, and staff about the treatment plan.
• Participate in discharge planning and care transitions.
