Job summary

Advance Hospice Care is seeking a dedicated Patient Care Manager to join our hospice team in Murfreesboro, TN. Reporting to the Executive Director, you will oversee patient care activities, coordinate services, and lead clinical operations to ensure compliant, high-quality hospice care.

You will mentor staff, facilitate IDG meetings, manage visit schedules, triage referrals, and ensure regulatory standards are met.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care experience, including 2 years in hospice or home-based care.
• Demonstrated ability to apply hospice principles, industry standards, and regulatory compliance.
• Knowledge and compassion for terminally ill patients and their families.
• Ability to lead a diverse care team with strong interpersonal skills.
• Competency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritization based on clinical judgment.
• Triage referrals and medical concerns promptly and professionally.
• Lead Interdisciplinary Group meetings and document minutes.
• Mentor staff and support performance and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support performance improvement initiatives and operational efficiencies.
