Qualifications

• Bachelor's degree in nursing or health care preferred.
• RN with a current state license to practice in the state of the office.
• At least 3 years of nursing experience, including 1 year direct patient care in hospice, home health, or oncology.
• At least 1 year supervisory experience in hospice or home health care preferred.
• Ability to manage terminal care with crisis intervention, pain and symptom control, and family counseling.
• Demonstrates a supportive, sensitive approach to staff and patient/family needs.
• Computer proficiency with EMR background.
• Current CPR certification if required by state.
• Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Coordinate and oversee patient services aligned with hospice program policies.
• Provide guidance and support to an amazing team of clinicians.
• Conduct chart audits to ensure regulatory compliance and ethical care standards.
• Participate in orientation and in-service training for nursing and clinical staff.
• Evaluate and uplift clinical staff performance during probation and annually.
• Lead interdisciplinary team meetings to refine Care Plans and share insights.
