Job summary

Hospice of Virginia is hiring an RN On Call Nurse to support Richmond and surrounding counties. You will triage after-hours calls, assess needs, and delegate nursing visits as appropriate, coordinating with physicians and the care team to ensure compassionate, patient-centered care.

The role requires an RN with strong clinical judgment, CPR certification, and the ability to manage family stress. A valid driver's license and insurance are required as you travel within the service area.

Qualifications

• Graduate from an accredited Registered Nursing program.
• Possession of a current license to practice in the state as a Registered Nurse.
• Minimum of two years of varied work experience as a professional nurse.
• Demonstrated skill in nursing practices and clinical judgments.
• Ability to cope with family emotional stress and tolerance of individual lifestyles.
• Must possess current CPR Certification.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Triage after-hours patient and family calls, determine services needed, and delegate nursing visits as appropriate.
• Gather data on physical, psychological, social, and spiritual factors affecting needs and coordinate with the interdisciplinary team.
• Coordinate care with attending physician and other agencies for optimal outcomes.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate records with care plans and progress notes.

What You’ll Do:

• Assume responsibility for triage of after-hours patient and family phone calls, determining the need for services including assessments, symptom management and care plan updates, delegating nursing visits to appropriate staff members
• Obtain data on physical, psychological, social, and spiritual factors that may impact patient and family’s needs and initiate intervention by other members of the interdisciplinary team as appropriate
• Initiate communication with the attending physician, other interdisciplinary staff members and other agencies for the purpose of coordinating optimal care.
• Perform clinical pronouncement of death and provide support to family
• Maintain an accurate clinical record through the timely preparation of care plans and clinical and progress notes
