Job summary

AccentCare is seeking a Hospice Registered Nurse (RN) to provide compassionate, patient-centered care in the Madison/Clinton/Old Saybrook area. You will assess needs, develop care plans with the interdisciplinary team, and support patients and families at the end of life.

This full-time role offers a schedule of Monday–Friday 8am–5pm, competitive pay ($90,000–$100,000) based on experience, and a comprehensive benefits package including medical, dental, vision, PTO, holidays, and a 401(k).

Qualifications

• Active RN license in state of practice.
• Minimum 1 year of nursing experience.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide individualized nursing care that maximizes comfort and quality of life.
• Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans.
• Perform comprehensive assessments (physical, emotional, social, and spiritual) to guide care decisions.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care clearly and accurately to reflect patient needs and progress.
• Participate in on-call or admission rotations as needed.
• Engage in ongoing learning and professional development.
