Job summary

AccentCare is seeking a Hospice Registered Nurse (RN) in Farmington, CT. The role centers on delivering compassionate, patient-centered care within an interdisciplinary team, while empowering you to practice at the top of your license.

This full-time position offers a pay range of $90,000–$105,000 per year, plus medical/dental/vision benefits, PTO, holidays, 401(k), and more. Schedule is Monday–Friday, 8am–5pm, serving the Litchfield and Hartford area.

Qualifications

• RN license in the state of practice.
• At least one year of nursing experience.
• Reliable transportation, valid driver's license, and auto insurance.

Responsibilities

• Provide individualized nursing care that maximizes comfort and quality of life.
• Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans.
• Perform comprehensive assessments—physical, emotional, social, and spiritual—to guide care decisions.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care clearly and accurately to reflect patient needs and progress.
• Participate in on-call or admission rotations as needed.
• Engage in ongoing learning and professional development.
