Job summary

Caris is seeking a Nurse Practitioner to join the Hospice Interdisciplinary Team in a PRN capacity. The NP will manage the medical component of the Hospice Patient Care Program, performing assessments, histories, exams, diagnoses, and treatment under the Medical Director's supervision.

Responsibilities include direct patient care in homes, nursing facilities, and hospitals, with collaboration with the Medical Director and Attending Physician. Benefits and flexible scheduling are offered.

Qualifications

• Graduate of a Master’s Degree Nursing Program with current ARNP license for the assigned state.
• Nursing experience required, hospice or home care preferred.
• CHPN certification preferred; strong liaison and communication skills.

Responsibilities

• Provide direct medical care to patients in home, nursing facilities and hospital under Medical Director supervision.
• Coordinate with Medical Director, Attending Physician, and hospice staff to maintain quality patient care.
• Liaise with community physicians and other health care providers to expand patient services.
