Caris-Healthcare seeks a Nurse Practitioner to join our Hospice Interdisciplinary Team, delivering medical care and certifications under the Medical Director. Responsibilities include home visits, nursing facility and hospital care, and collaboration with physicians and staff to maintain quality patient outcomes.
The role requires a Master’s in Nursing with ARNP licensure; Hospice/Home Care experience is preferred. Strong communication and a proactive liaison with the care team are essential.
Pay: $100 Per Visit
The Nurse Practitioner serves as a member of the Hospice Interdisciplinary Team and has responsibility for the medical component of the Hospice Patient Care Program. Collaborates with Medical Director and Attending Physician to provide skillful certifications and recertification assessments, including physical examinations, medical history & physical, diagnosis and treatment prescribing medications as needed, under the supervision of the Medical Director.

Nursing experience required, with experience in Hospice or Home Care preferred.
Training, experience, interest, and knowledge regarding the psychosocial, and medical aspects of terminally ill patients is required; CHPN certification preferred
Self directed; excellent communication, organizational and interpersonal skills; ability to act as a liaison with team and physicians.
