Job summary

PruittHealth Hospice is seeking a Nurse Practitioner to join our hospice team, delivering compassionate care in patients' homes or care facilities. We offer top‑of‑market pay, flexible schedules, and comprehensive health plans to support your nursing career.

Responsibilities include conducting comprehensive adult/geriatric assessments, managing care plans, and ensuring timely documentation. AANP/ANCC board certification and active NP licensure are required to practice in the state.

Qualifications

• Current, active, and unrestricted RN licensure in state of practice.
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.

• Proactive, collaborative team member in a hospice care environment.
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care.
• Commitment to caring for patients and partners.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
