Qualifications
- Must hold a current State RN license and an Advanced Practice Nurse license.
Responsibilities
- Collaborates with the IDG to assure appropriate evaluation and palliative care for terminal disease.
- Educates patient and family regarding disease, self-care techniques, and end-of-life care.
- Performs Face-to-Face visits at the patient's place of residence.
- Interacts with attending and consulting physicians regarding palliative care.
