Job summary

Caris, an affiliate of NHC, is seeking a Nurse Practitioner to join the Hospice Interdisciplinary Team on a PRN basis in Charleston, SC. The role involves medical component of the Hospice Patient Care Program, certifications and recertifications, and prescribing medications under Medical Director supervision.

Under Medical Director guidance, you will provide direct medical care in patients' homes, nursing facilities or hospital settings, collaborate with physicians, and contribute to quality

Qualifications

• Master’s degree in Nursing with ARNP license for the assigned state.
• Hospice or home care experience preferred.
• CHPN certification preferred.

Responsibilities

• Under Medical Director supervision, coordinates with MD, Attending Physician, and team to ensure quality patient care.
• Liaises with community physicians and other clinicians to expand patient access to hospice services.
• Provides direct medical care in patients' homes, nursing homes, and hospitals as needed.
