Job summary

Interim HealthCare Inc. is seeking a Hospice and Palliative Care Nurse Practitioner (NP) to provide compassionate, patient-centered care to our clients.

You will perform bedside evaluations, contribute to diagnoses and management, and help ensure comfort and dignity at end of life. This role requires current Advanced Practice Nurse and RN licenses, a valid Controlled Substances license, and hospice or palliative care experience is preferred.

Qualifications

• Licensure: Current Advanced Practice Nurse (NP) and RN licenses in the state.
• Controlled Substances license required.
• Hospice or Palliative care experience preferred.

Responsibilities

• Perform bedside palliative care patient/family evaluation and assist with diagnosis, treatment and management of acute and chronic conditions.
• Consult with palliative care physician or designees, inform primary physician of services provided and collaborate with other physicians as needed.
• Prescribe medications including controlled substances to the extent delegated and licensed.
• Prepare and maintain accurate patient records, charts and documents to support sound medical practice and reimbursement for services.
