Job summary

Interim HealthCare Hospice & Palliative Care is seeking a Nurse Practitioner to provide compassionate hospice and palliative care across patient settings. You will conduct bedside evaluations, assist with diagnosis and treatment, coordinate with palliative care physicians, and maintain accurate records to support quality care and reimbursement.

Qualified applicants hold Advanced Practice Nurse and RN licenses in the relevant state, possess a Controlled Substances license, and ideally have

Licensure as Advanced Practice Nurse and RN in the relevant state.
Controlled Substance license required or licensed.
Hospice or Palliative care experience preferred.

Responsibilities

Perform bedside palliative care patient/family evaluation and assist with diagnosis, treatment and management of acute and chronic health conditions.
Consult with palliative care physician or designees and inform primary physician of services provided.
Prescribe medications including controlled substances to the extent delegated and licensed.
Prepare and maintain accurate patient records, charts and documents to support sound medical practice and reimbursement.
