Job summary

Amedisys is seeking a Nurse Practitioner for PRN home health/hospice coverage in Missouri. The NP will assess patients at home, document care plans, and collaborate with the interdisciplinary team to manage symptoms and care transitions.

Qualifications

• Must hold an active Nurse Practitioner license in the assigned state.
• Minimum of 1 year NP or hospice RN experience in the last 3 years.
• Must have Medicare billing privileges as a Nurse Practitioner.
• Must have current CPR certification.

Responsibilities

• Communicate significant findings or changes in patient condition to the clinical team.
• Conduct face-to-face encounters and assess disease progression, function, pain, and quality of life.
• Order and review labs, imaging and diagnostics as appropriate; document treatment plans.
• Make referrals to other disciplines as needed and participate in team conferences.
