Job summary

Bristol Hospice seeks a compassionate Nurse Practitioner to join our hospice team in delivering patient-centered care to adults and children with terminal illnesses. You will work under physician supervision to provide palliation, manage clinical plans, and coordinate comprehensive hospice services with an interdisciplinary team.

The role requires a NP with a nursing license in the state, a master’s degree, and hospice/palliative care experience; CHPN or related certifications are preferred.

Qualifications

• Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
• Must have current nursing licensure in State.
• Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
• Certification in a specialist area preferred (e.g., CHPN, Pediatrics or Geriatrics).

Responsibilities

• Provide medical care and palliate/manage terminal illness under the supervision of a collaborating physician.
• Complete history and physical exams, order/interpret tests, document findings.
• Develop an individualized plan of care and coordinate hospice services.
• Review and coordinate patient care with the hospice interdisciplinary team.
• Ensure symptom control and collaborate on treatment protocols.
• Consult collaborating physician when plan falls outside standard practice.
• Fulfill assigned tasks and participate in team meetings.
